Our dedicated Transitional Care Unit at The Manor Health Care provides a bridge between hospital and home, offering excellent rehabilitative care in an environment that is dedicated to quick recovery and returning home as quickly as possible. Morningside Transitional Care’s Pulmonary Rehabilitation Program provides patients with a comprehensive respiratory assessment and a complete physical evaluation upon admission. An individualized plan of care is created by our interdisciplinary team based off the physician’s recommendations and protocols.
